3 Approved forms For the purpose of section 1.1.7 of Defence (Employer Support Payment Scheme) Determination 2023, as in force from time to time, the approved form for the following actions is the form in the Employer Support Payment Scheme online claim system. a. Applying for a support payment — section 3.2.1 b. Applying for an extended support payment — section 4.2.1. c. Applying for an additional support payment — section 5.2.1. d. Requesting a review of a reviewable decision — section 7.2.2.
