(2) the respondent is disqualified from being registered as a pharmacist for a period of 3 years from this date (3) we ask the Registrar of this Tribunal to request the Pharmacy Board of Australia to record the fact that the Tribunal would have cancelled the respondent's registration in the National Register kept by the Board (4) costs are reserved with liberty to apply which must be exercised within one month of this date.
