The practitioner's name be forthwith removed from the Register of Medical Practitioners.; 2. The practitioner be not permitted to apply to be re-registered for a period of three years from today.; 3. Any application by the practitioner to be re-registered be made to the Medical Board in the first instance.; 4. The practitioner pay the costs of the complainant.
